Village Medical is a healthcare company that provides full-service primary care to patients at traditional free-standing clinics, Village Medical at Walgreens clinics, at home, and through telehealth.

Chosen Family Medicine is an LGBTQ+-focused primary care practice in East Nashville serving roughly 2,200 patients, more than half of whom identify as transgender or gender-diverse. Board-certified family physician Dr. Hayden Shafer, a founding provider of UNC's Transgender Care Program, offers comprehensive primary care, gender-affirming hormone therapy, and preventive health services. The practice uses an informed-consent model for hormone therapy and maintains a culturally competent, affirming environment.

Clinica Family Health is a community health center that provides basic health care to low income and uninsured people in south Boulder, Broomfield, and west Adams counties.

Corktown Health is Michigan's first and only LGBTQ+-focused medical home, offering primary care, HIV care and treatment, gender-affirming care, PrEP and PEP, STI testing and treatment, and behavioral health services. The clinic actively trains the next generation of LGBTQ+-affirming clinicians in partnership with Wayne State University and the University of Michigan. Corktown accepts most insurance plans and provides services regardless of ability to pay.
